2nd Phase: - This is the validation exercise where we set up a clinical team or task order team from the organization or facility; train them on this tool to sustain this process by always looking out for further program gaps or clinical errors, resolving it by themselves and then eliminating it from happening again.
3rd Phase: - At this phase, we review the internal strategic policy guideline of the program or your facility to capture the findings of this exercise as a rational to
validate it as an internal practice in your healthcare program or facility in line with International standards.
